Information we will collect
The application form requires survivors to submit essential information to enable the identified redress agency/agencies (Ministry of Social Development (MSD), Ministry of Health (MOH), Ministry of Education (MOE) and Oranga Tamariki (OT), to reach out to the survivor and begin processing their top-up application.
Information requested is as follows:
- Full name
- If it is different now, what name was your claim resolved under?
- Date of Birth
- Preferred method of contact to be kept up to date on the progress of this application (e.g. phone or email)
- Email address
- Phone number
- Any days or times where the applicant does not want to be contacted
- Government department(s) that you received a payment from
- For those applying from prison, which prison are you currently in?
What the collected information will be used for
Collected information will be used to ensure top up applications are provided to the correct redress agency/agencies that had previously provided them with a payment as a result of a resolved claim.
The information will also be used to assist agencies to make contact with the survivor, confirm eligibility and process the top-up payment.
How we will look after your information
As host agency for the top-up application process MSD is responsible for ensuring that personal information collected through the top-up application process is stored and retained in accordance with MSD’s privacy policy and information policies, protocols and guidelines, and in compliance with the Privacy Act 2020 and the Public Records Act 2005.
Access to information
Survivors can request copies of information held regarding submitted top-up applications to MSD by emailing, contact@abuseincaretopups.govt.nz.
Correction of information
If a survivor believes that the information held is incorrect, they have the right to submit a request for the information to be corrected by:
- if you are in New Zealand, calling 0800 TOP UPS – 0800 867 877 between 8am and 5pm Monday to Friday
- if you are calling from overseas, calling +64 4 931 1678
- emailing, contact@abuseincaretopups.govt.nz.

Complaints
Once a top-up application is received it will be sent to the relevant redress agency to confirm eligibility and to process the application. Complaints about that particular redress agency’s personal information management can be made by contacting the specific agency:
